This game has rail fan's favorite game mechanics, from openable windows and lots of toggleswitches in the cab of the locomotive to performing maintenance and replacing worn parts on your locomotive to wear mechanics to mafia missions to shoot all of the gang members in a strip club to drunk driving to theft mechanics.

I'm a bit split on this game: On one hand, it connects a genre that I should like but don't (Train Simulators with realistic train controls) with a genre I don't want to like but do (Story RPGs) and does that incredibly well.
