I know I'm forgetting some stuff, but so far, I've noticed: five characters (two new), a card enchanting system, "epochs," new bosses, new Acts (with possibly more than three?), new enemies, new events, new relics, new potions, new cards, new status effects, nicer graphics, new animations, new modifiers, better events, more lore, better rebalancing, run history (on each floor!), and multiplayer.

It is a no-brainer buy though if you have friends who are also into deckbuilders, thanks to the addition of co-op.
They offer 3 options for you to choose from at the end of each Act, which adds much more gameplay variety compared to simply gaining 1 energy with a downside like in the first game.
