It transforms colonoscopy into an eerie form of exploration while combining medical interactions, retro graphics, disturbing imagery, branching outcomes, and surreal storytelling.

Most of its appeal comes from atmosphere, unusual interactions, narrative escalation, and the anticipation surrounding each examination.

The game uses old-school graphics, which works really well in this scenario, since going through a dark fleshy pixelated tunnel successfully creates a feeling of discomfort and claustrophobia that I feel we would not get with an anatomically accurate colon with RTX and all those other fancy graphical innovations.

Some of the unexpected sound effects in the later stages also worked pretty well, I won't go into spoilers, but obviously the game takes a few weird turns, I think you navigated the mood about as well as you could.
