I'm utterly deranged and played this without the patch, which as you might expect, guts a lot of the content like character art, entire quests are broken or removed and areas made inaccessible.
There are some minor plot points that never come to a conclusion in some of the side quest ( or I couldn't find a continuation to some of them at least ) but neither the bugs or plot holes affect the progression of the games main story assuming you're not killing anyone important to the main quest.
